Cook County Vital Records are available by visiting a Family Search Library or Family Search Affiliate Library. The following collections are available and have images available for library patrons. Please use the links below to locate the record(s) you would like to request.

A pre-paid retrieval fee of $10.00 is required per vital record. Please provide the index information from database(s) above. Typical turnaround time for these requests is one week.

*A gap exists in records for the City of Chicago deaths. While they are indexed online the images are not available from approximately 1950 to 1960. Deaths for other areas in Cook County are available for this time period. Other gaps may exist; however, are not known at this time. If a record is not digitized a partial refund will be issued.

Cook County Coroner's Inquests, 1872 through 1911, and Certificate Certificate of Death, 1879-1904, have also been digitized and made available for library patrons. Please use the link below to search the inquest database.

A pre-paid retrieval fee of $12.00 is required per coroner's inquest. Please provide the index information from the database above. If the inquest record overlaps with the certificate of death both records will be retrieved. Typical turnaround for these requests is one week.
